It has been told in the investigation report that there was no lapse of the pilot and co-pilot and guard of the train in the accident. Together, both of them made every effort to save the train from their respective places. In such a situation, now the technical test of the engine of the train concerned will be done, only then the reason for the brake failure will be clear. Technical test of the locomotive is not possible at the site.
Coal spread on both sides of the track was causing trouble in the operation.
It has been told in the investigation report that there is a possibility that the train derailed due to failure of brakes of the vehicle. After this, the column responsible for brake failure has been fixed as (1) Carriage and Wagon Department, BPC issuer and (2) Locoshed Kanpur.

A total of 20 points have been mentioned in the investigation report. At present, the loss on the spot has been estimated to be more than Rs 12 crore. Also, it has been told in the investigation report that according to the written statement of the pilot and guard to the loco pilot, the train opened from Hazaribagh Town at 12:20, stopped at different stations, stopped at Gajhandi station in the up second loop at 4:22 and then Opened at 5:55 am. After which he got the home signal check of Lalbagh station and passed it at 6:05 am.

Everything was fine till now, but after moving ahead from Lalbagh, pressure was dropped to control the speed on the slope but the speed control was not done. The speed kept increasing. The loco pilot and co-loco pilot applied the emergency brake but the vehicle was not controlling. The guard was then asked to apply the brakes through the walkie talkie and through three small whistles. Also the flashlight was lit. Meanwhile the red flag was shown to the station. All right signal was also exchanged by the station master. After this the guard dropped the pressure from the brake van but still the speed of the vehicle did not decrease. Signals were up-through at all stations on the way. But the up home signal of Gurpa station was red. Meanwhile, the train went into the loop line and in one stroke 53 bogies got derailed.
10 cranes, more than 20 JCBs were engaged in clearing the track day and night.
112 kilometer per hour has been found
The investigation starts from the station master’s panel room. The point set for the up-loop line and run-off line has been found on the panel. At the same time, the up home signal of Gurpa station was passed by the loco pilot in red position. This was also found in writing. Apart from this, the up loop of Gurpa station was track feet for a speed of 30 kmph but due to brake failure and slope, the speed of the train has been found to be 112 kmph as per the speed chart. Because of which this big accident happened.
This investigation report has been prepared by seven officers of different departments of the Railways. The inquiry report has been sent to Headquarters. The investigation report also states that the engine speed is restricted to 100 km.
